1. Each new client of Lege Artis Counselling has to fill out an INTAKE FORM and send it by email.
2. Before we start, please fill out and send to us Counselling or E-mail Counselling Agreement. *You could submit your Electronic Intake Form and Agreement for E-mail Counselling here.
3. If you are referral client from Health Care Professionals, you can print out and ask your practitioner to fill out our  REFERRAL FORM, or you can ask your practitioner to fill out an ELECTRONI C REFERRAL FORM on our website.
4. If you like to pay for counselling with your extend insurance, but your insurance does not cover it, here sample of LETTER to your Insurance Company.
